Health/Medical Form

If you are a Guild Member attending an in-person event such as a workshop, sew day, or retreat, prior to the retreat we ask you to please print and fill out a copy of the Guild medical information form (click on the button below), seal the completed form within an envelope, and print your full name on the outside of the envelope.  Bring the sealed envelope with you to the event and provide it to the event coordinator (at the start - prior to sewing).  The envelope will only be opened in case of emergency, and if unopened will be returned to you at the end of the event.

Costs and Payment Options

Cost

The total cost is $175.00 per person.  There is no reduced rate, for attending fewer days.

To hold your spot (and to cover non-refundable retreat expenses, per the instructions of guild membership to executive at a former AGM), a deposit of $50 deposit is required to be sent by e-transfer at the time of registration.  This deposit is considered to be non-refundable but is fully transferable.  In the case of extenuating circumstances resulting in a cancellation, the retreat convenors (president or secretary) will do our best to assist you.  You can contact them by emailing the guild at dtqguild@gmail.com.  You can pay the full amount at this time if you desire, but the deposit is required to reserve your spot.

Full payment (so an additional $125 if only $50 deposit was initially paid) is required to be submitted by etransfer by 2025 August 31.  If the retreat is not filled at this time, then in September it will be opened up to non-members.

Massage Option

Note: pre-arranged massages are available for Saturday only (so when you sign up on the form please indicate if you would like a 30  minute hand and neck massage for $65, or a 45 min massage (more tailored to what hurts you (from head to toe) for $85. Becky the registered massage therapist will give you a receipt so you can provide it to your health care provider, should you wish to do so.  If you take advantage of a massage please remember that your massage cost is above and beyond your cost for the staycation retreat.  Massage payments are to be made directly to the massage therapist on the Saturday.

In case more people want a massage than time allows, appointment slots will be filled on a first registered (and paid deposit for retreat) first signed up basis.

Please note that a minimum of 24 participants are required in order to run the retreat, and there is a maximum attendance of 30 participants.  The order of registration will be based on the submission date and time of this electronic form and payments received on time.

You MUST be a registered and paid member (for 2025-2026) to register to attend the retreat.  Please ensure to register and send in your membership payment prior to registering for the retreat.
